Parents are regularly in need of personalized letters to judges, attorneys, insurance companies, the IRS as well as other agencies or individuals.

Providing our patients with a variation of correspondence is a challenging task. These documents at times are quite extensive and consume time away from our physician seeing patients.

Letter Processing and Fees
We have simplified the process by establishing simple steps that we believe will serve our clients best. Management has prepared basic form letters of routine matters that can be prepared within a short period, for a small fee. Please contact our staff for this fee schedule.

As payment is required in advance for fees that apply to physician letters, we will gladly take a credit card payment by phone, or you may stop by the office to request the service. We will call you when the document is ready for pick-up.

Letters and forms to be completed outside the basic form letter can be prepared, and the fees for these specialized documents may vary. Contact our staff for specific information. Please note the physician is not obligated to attest in any letter or form to statements that are not supported in the patient's history.

Please allow three business days for processing of your request; however, production of an order can be expedited for an additional rush fee of $25.00.
