Medical Records
 Records Request Processing Period
The laws of the State of Florida and federal government require that request for medical records be submitted in writing, with a signature from the child's parent or legal guardian before any records can be released. We try to process requests for medical records within 10 business days, however, at times we may take as long as 30 days, the extent of time allowed by law.
Fees Involved
We charge a fee for documents copied and released to other parties such as family members, attorneys or insurance companies. There is also a charge for records requested by a parent when records have been supplied more than once within a six-month period. Extra Blue and Yellow forms given beyond the complimentary set provided at the time of your child's annual well exam are also provided with a service fee. As payment is required in advance for fees that apply to copying of records or producing Blue and Yellow forms, we will gladly take a credit card payment by phone, or you may stop by the office to request the service. Our staff will call you when the documents are ready for pick-up. Please allow three business days for processing of your requests.
Transferring of records
There is no fee for records to be released to another treating physician. If you choose to transfer the primary care of your child to another physician, please note that we maintain documentation of where records are transferred to and fax confirmations of their receipt. Should you transfer your child again, after we have already forwarded your child's records, you will be directed to obtain all records thereafter, from your most recent attending physician to whom we transferred your child's records.
Bring your child's shot records to your first visit.
When bringing your child to our practice for their first visit, it will be necessary for us to have a record of all your child's immunizations, before we are able to administer new vaccines. This will prevent unnecessary, costly and possibly harmful duplication of immunizations. In cases where the previous medical record is not in the parent's possession, we will gladly request them from your previous doctor. Arrange this with our staff before your visit. Please go to our patient forms page and print the request form for your child's records to be obtained by our practice.
Remember to take records with you.
Likewise, if you are transferring out of our practice to another physician, do obtain an updated copy of all vaccines administered through our practice, well in advance of your first appointment at your new practice.
