Sports/Camp Physicals
 Kids Should Play Healthy
Many organizations require children to have a current annual physical exam prior to participating in their activities. Sometimes they require certain lab tests and immunizations. Do bring the required forms with you for our staff to complete upon your visit.
There Is A Timeline That Applies
Often times, if your child has had a preventative care or well visit with our practice within the previous 11 months, these requirements have been met. In this case, you will only need to schedule a sports/camp physical, which is a brief check-up, not as extensive as a well exam, but is sufficient for our physician to ascertain along with any previous medical history if your child is able to participate in the sport of choice.
A Non-Covered Fee
These sport/camp physicals, though required by many organizations, are not regularly covered insurance benefit for many plan, therefore, this fee is out-of-pocket to the patient family. Please contact our staff for the sport/camp physical fee.
If your child is due for a physical, the sport/camp physical fee is waived, and an annual well exam will be performed instead; charges for a regular well check-up will apply.
Remember To Bring Forms
The organization overseeing the sport activity should supply you with the forms they need our physician to fill-out. Do bring the required forms with you for our staff to complete upon your visit. If you do not have the forms prior to your scheduled visit, please call and reschedule the visit, or call the organization to request they fax it to our office before your child's appointment.
